Accident Summary:

AIRCRAFT 67-17376 WAS FLYING A LOG MISSION FOR THE 2/8TH CAV OUT
OF PHOUC VINH, RVN. THE AIRCRAFT HAD BEEN FLYING FOR 11 HOURS
WHEN THE CREW AND AIRCRAFT WERE RELEASED FROM THEIR MISSION.
AT 1700 HOURS THE AIRCRAFT WAS REFUELED AT PHOUC VINH AND HEADED
DIRECTLY TO DAU TIENG WHERE CO B, 229TH AHB IS LOCATED.
DURING THE COURSE OF THE DAY THE AIRCRAFT HAD BEEN WORKING
CONTINUOUSLY IN WHICH THERE WHERE MANY AREAS THAT HOVER DOWNS
FROM 200 FEET TO 100 FEET HAD TO BE DONE TO GET RESUPPLIES
TO THE GROUND TROOPS.

THE AIRCRAFT DURING THE DAY WAS REPORTED NOT TO HAVE ANY
MALFUNCTIONS OR LOSS OF POWER. ABOUT 10 MILES EAST OF PHOUC VINH
IN ROUTE TO DAU TIENG AT 1800 FEET AGL THE ENGINE CHIP DETECTOR
LIGHT ILLUMINATED. THE ENGINE OIL TEMPERATURES INCREASED FROM 90
DEGREES TO 95 DEGREES AND THE EGT WAS ABOUT 580 DEGREES,
THEN A FEW SECONDS LATER THERE WAS A LOUD BANGING SOUND FROM
THE ENGINE, THE AIRCRAFT SHUTTERED AND THE ENGINE INSTRUMENTS
FLUCTUATED. THE AIRCRAFT COMMANDER REDUCED PITCH TO TAKE STRAIN
AWAY FROM THE ENGINE. THE INSTRUMENTS RETURNED TO NORMAL.

A FEW SECONDS LATER ANOTHER LOUD BANGING SOUND OCCURRED,
THE AIRCRAFT SHUTTERED AGAIN AT 1200 FEET ALTITUDE AND THE
INSTRUMENTS FLUCTUATED AGAIN. THE ENGINE RPM BEGAN DETERIORATING
AND THE AIRCRAFT COMMANDER ENTERED AUTOROTATION. DURING
AUTOROTATION THE AIRCRAFT COMMANDER TRIED ROLLING ON THE ENGINE
BUT DID NOT SUCCEED. THE AIRCRAFT COMMANDER INCREASED HIS AIRSPEED
FROM 80 KNOTS TO 100 KNOTS IN AN ATTEMPT TO REACH THE FORCE
LANDING AREA. MR. STANTON SAW HE COULD NOT MAKE THE AREA,
SO HE FLARED AT THE TREE TOPS TO REDUCE AIRSPEED AND PULLED
IN PITCH IN AN ATTEMPT TO DECREASE HIS RATE OF DESCENT BEFORE
IMPACTING WITH THE TREES. THE AIRCRAFT ROLLED AND LETTLED TO THE
GROUND FROM TOPS OF THE TREES (APPROXIMATELY 50 FEET) ON THE
EDGE OF THE CLEAR AREA THE AIRCRAFT COMMANDER WAS TRYING TO REACH.

THE CREW EVACUATED THE AIRCRAFT AND ANOTHER SHIP HEARING THEIR
MAY DAY CALL PICKED THE CREW UP. THE AIRCRAFT COMMANDER SUSTAINED
ABRASIONS ON THE LEFT LEG. THE PILOT SUSTAINED A FRACTURED JAW,
FRACTURED RIGHT ARM AND FRACTURED HIP. CREW CHIEF RECEIVED
ABDOMEN INJURIES AND GUNNER HAD FRACTURED RIBS ON THE RIGHT SIDE
AND HIS LEFT HIP WAS FRACTURED.\\
